HITH was launched in August 2023
Hospital in the Home (HiTH) is an acute inpatient service, providing treatment for patients with a medical condition which requires ongoing care not suitable to be managed in an existing community / outpatient setting.

Through the HiTH service, treatment can be provided:
- in the patient’s home and/or
- at our clinic located in RADIUS on Level 2
- To be eligible for treatment a patient must:
- be 18 years of age or older
- reside or be staying in the Sutherland Shire LGA
- have a suitable/safe location for care outside the hospital for the patient and HITH staff
- have consented to HITH treatment and care in their home
- not require regular observation/assessment or treatment in a 24-hour period
- suitable for daily or twice daily interventions only
- be cognitively intact (or have a competent carer present) in order to manage their condition and escalate their care if required
- must have reliable telephone access and be contactable at all times
Referrals can curently be made in hours (0730 -1600) to the HiTH Care Navigator via switch or email SESLHD-TSH-HITH@health…- After hours, referrals must be made & accepted by the General Medicine consultant
- If consent is obtained for patient to be transferred to HITH, referring medical team to complete patient discharge summary and discharge medication reconciliation prior to transfer of patient to HITH.
- Referring medical team is to ensure that only the medications to be administered by the HITH team are left on eMEDs and all other medications are ceased within eMEDS on transfer to HITH.
- Please ensure clerical staff transfer patient to HITH (i.e. the patient is not to be discharged home from FirstNet)
