• Medical treatment of acutely unwell residents in aged care facilities (RACF)
  • Monday – Friday 0800 – 1630
  • Referral must be made by RACF’s GP
  • Recommend GFS referral in your discharge letter and include a GFS flyer (found in the pigeonholes in the southside of the Acute workstation)

To avoid transfers from RACF to ED:

  • GP referral to GFS will lead to review at the RACF within 2 to 4h  (in-hours)

To avoid admissions from ED to ACAU:

  • Where Southcare nursing or specialist geriatrician input is required acutely and can be provided in the nursing home
  • NOT for routine review of nursing home patients discharged from emergency

Southcare nurses provide ongoing clinical treatments (e.g. IV abx, monitoring of fluid overload)

If patient deteriorates, Flying Squad can arrange direct admission to ACAU
