Community nurses
- Cellulitis: IV antibiotics at home
- DVT: clexane, warfarin, compression stockings
- Pneumonia: IV antibiotics
- General community nursing e.g. dressings, catheters, drains
- Referrals are made in eMR
- Other paperwork will also be required e.g. scripts, med charts
- Get assistance from an experienced Sutherland ED doctor or nurse practitioner to complete the required paperwork correctly
SOS: SouthCare Outreach Service
Do you have a >65 year old client who is likely to re-present if not seen by a Nurse, Physio or Occupational Therapist quickly?
Once a medical follow-up plan has been established for discharge then please refer to the Southcare Outreach Service (SOS) via eMR for review of the patient in their own home.
Referrals are accepted from ED staff for the following:
- Clients over 65 years of age
- reside in the Sutherland Shire
- NOT being admitted to hospital
- at risk of re-presenting to ED
- goals that can be addressed by a community rapid response multidisciplinary team at home
- NOT currently receiving high level care in a RACF (Residential Aged Care Facility)
Patients can also self refer (for example, if they are not certain they want to be referred at the time of their ED visit). Please provide the patient or carer with a brochure. Brochures can be located in the pigeon holes in the Southside of the Acute work station.
RCCP: Respiratory Chronic Care Program and SHALT: Sutherland Heart and Lung Team
For home-based management of exacerbations of e.g. COPD or CCF
- Patient needs to be known to a TSH respiratory physician or cardiologist.
